LEAP Brochure Judging Process Submission Instructions Application Form

The LEAP Award was created by Baxter to recognize outstanding contributions to the practice of pharmacy in which a health care provider implemented a LEAN initiative using at least one of the Baxter drug delivery products (MINI-BAG, VIAL-MATE, MINI-BAG Plus, Liquid Premix and/ or Frozen Premix Medications) that provided his or her health care setting with a reduction in some form of waste as defined by LEAN.

This award consists of a contribution to the charity** of your choice in the amount of $750 for the Bronze award, $1000 for the Silver award, and $1500 for the Gold award. Ten (10) recipients will also receive a plaque inscribed with their name and title of their winning entry, a brief description of their winning entry on Baxter’s Pharmacy Advisor web site, and an ASHP awards reception for all recipients at the Marriott Renaissance Pere Marquette Hotel in New Orleans.
For this award, pharmacy practice refers to all aspects of professional practice in hospitals and health care systems, chronic care, ambulatory care setting and home health care providers, including administrative, managerial, drug-use control, technological services, clinical services, information processing, and new practice models.
At least one of the applicants submitting a LEAN nomination must have held the primary role in the initiative as a pharmacist. If an application is submitted by one person, then that person must be the pharmacist who had the primary role for that LEAN initiative.

**Only 501(c)(3) Charitable organizations are eligible to receive the award. Applicants can have no direct relationship with the charity they are nominating to receive the award. Applicants cannot work for any organization affiliated with the charity nor have any family members who work for that charity, organization, or Baxter Healthcare Corporation.
